I'll try to list every effect and the easiest way to get them so people won't have to ask in the threads and waste time and everyone's patience. All directions are from the entrance of the area. All area names are completely made up by me, unfortunately.

Yume 2kki v0.088 (15 effects):
1)Bike- Flower Field: Go slightly SE
2)Wolf- Dream House: Open closet in room repeatedly
3)MedamaBomb- FC Maze (maze in Block World): See Map
4)Twintail- Guts World: Go slightly W, then N
5)Boy- Wire World: Go W until there is a vertical line. Follow it N into a room. Interact with the thing in the room until you can't. Go outside and interact with the boy NPC.
6)Lantern- Cave: From Cloud Forest, go SW one block of trees, then follow the left block of trees S.
7)Maiko: Graveyard- Go slightly E, then N
8)Chainsaw- Hospital: From Graveyard, go NE to Dead Heart and go through mouth. Go W into hospital. Go through 3rd to last door to an area with no floor. Go S. until you hit a wall, and then go W to a dot. Go into NE Dead Spade(?), then N into nearest Dead Diamond.
9)Fairy- PKMN Park: From Blob World, go S until you see a waving blue thing. Go E to a Cab, which takes you to the Highway. From Highway, go E until you're in a tunnel. Chainsaw the face splat on the wall and go through. Take the S. path in the next area to get to the PKMN Park. Go to the bottom of the large area (not the bottom of the map) and there will be a black creature. DON'T GO UNDER IT. Step on the flowers and go E into a grove with a fairy in it.
10)Telephone- Girl's Room: From Goddess World, go west to an electrified thing. Go N into the Auditorium's Lounge. Go 2 rooms S into the Girl's Room.
11)Rainbow- Sewers (lower part): From Auditorium's Lounge, go to room with the 3 eyed guy and go E to the Sewers. Follow the (mostly) linear path to Japantown. Go W until you're in the room before the Bar. Go below the stairs and go E to the lower Sewers, and the Rainbow.
12)Glasses- Museum: In Japantown, there is a red Postbox by the Blue Guy's House (the last one before the bar). It teleports you to the Museum. Go E to the main hall of the Museum and take the right hole in the wall (not the rightmost path).
13)Space Suit: Orange World: From Museum's main hall, go S until you're in the Orange World. Take the middle door to go to the ground floor. Go S until you can't, then E until there's a building with a graph on it. Go up the building for the suit.
14)Bug- Bug Temple?: From Goddess World, go S into Recursive Creature. Go into it's left (your left) ear's warp into the Bug Maze. Make your way to the large flower to the Countryside Mountains. Chainsaw the thing in front of the rock and lure it away and touch the rock to get to the temple...ish place.
15)Penguin- ???: ???

Yume Nisshi v.whatever's out (5 effects):
1)Match- Snow World: go SW a bit
2)Doll- Doll World: from Desert World, go N into Guts Maze. Take the right branch (there's only one split) to the Doll World. Go S until you see a giant doll with no face and go E to a similar one, and then go S directly between the two.
3)Braids(?): Smokestack Top: From Dark Maze, take either path (they form a loop) and take the S branch out of the loop. Take E branch immediately after, and touch the vines to get to Sky Streets. Go S to an intersection and go W, and W again at the next intersection. Follow the street to a crack in the ground, which leads to the Underground Smokestacks. Go S until you see a lone smokestack, then go E until you see a giant one with an entrance. That leads to the Smokestack Top, and touch the cable on the giant thing for braids/pigtails/I don't know.
4)Pyramid Head- From Sky Streets's 1st intersection, go E, then S, then E 2 intersections, then N to Blood Streets. From there, Go S, then E 2, then S 3, then W 1, then N, dodging hostile NPCs along the way.
5)Club- Church/School: See >>2373

.Flow v.007 (9 effects)
1)Pipe- Pole World (West World): Go SW
2)Witch- Witch's Hut: From Pipe effect, Go S (maybe W a bit) to a box w/ a hole. Enter box and go down path to the Apartment. Go E to the other set of stairs, and go up them. On the roof, there will be a block which leads to the Square Maze. Take the N path all the way and go over the grate(s) to the FC World. Go N to the Witch's Hut.
3)Headphones- Sound World (East World): Go S until you see red boxes, then E to find a DJ.
4)Whistle: Flesh Throne: From 'Phones, go N to the Giant Medamaude pattern, then E to a group of boxes. There's a pair of things in the middle. Go between them to go to a path. At the other end is the Hospital Maze, which is sort of random. You'll get to a room with 4 directions. Go down the path with the most blood to get to a room w/ 4 things and a doorway w/ lots of blood. Go through that to the Red map. Follow the patterns N, then W 2, then N to get to the Blood Monster. Go through its right mouth to get to the Innards. You'll notice a hole in the wall with a harry thing next to it. Go through the hole to the Flesh Throne, where the whistle is in plane sight.
5)Ghost: Ghost's Garden: This one's long. From the Blood Monster take the asterisk to the Black Maze (only fast if you just got the whistle. Otherwise, go to the apartment and take the elevator to the Black Path. Go E, looking for a path N, then keep going N to get to the Black Maze) From where you're dropped off, go S, W, S (to the normal entrance), E to a second set of stairs. Take those to the Flower Marsh. There, keep finding the white flowers in every area to get to then next one. Eventually, you'll find the Vine Cave. Go to the end of it and touch the flower, then exit. Go to the warp flower outside and it warps you to the Ghost's Grove.
6)Watering Can- Ghost's Garden (South Area): You'll be warped right next to the NPC w/ the watering can if you exit the Ghost's Grove. Otherwise, from the entrance, go S to a large group of flowers, then W to the correct NPC.
7)Diver's Helmet- Diver's Hut: From Ghost's Garden, go S to the large group of flowers, then go E to a pair of pillars. Go between them to go to the Water World (alternately, fail the Hospital Maze enough and touch the screen in the room. Then go E and go through the rocks). Go S to the Deep Water Field. Go E to a rock formation, then S until you see a building on your right. That's the Diver's Hut. You can only get its effect if it's working on the machine thing, I think.
3)Cyclops- Eye&Mouth World (North World): Go N to a set of 3 straight, eyeless things, then go E to 3 things around an Eye. The eye's your goal.
9)Cat- Rainbow Pond: This is long, too. From Cyclops, go N a bit to find some hands. Go through them to enter the Hand Cage. Step on the Paw prints in there to go to the Tentacle Monster's Path. Go E and follow the footprints N to get to Pong Path. Go through it and you'll be regurgitated into Rainbow Maze's Entrance. After you step through the door, you'll be in the Initial Maze. Go E and use the first door you see. That gets you to the Main Maze, which is far more complex. Go W, then the southern W path, and keep going W until you see a door. That leads to the Rainbow Pond (South), where you'll find the Cat.

Hallucigenia v.0.003 (4? effects)
1)Cockroach (Chairo)- Church Backstage: Double doors = Church. Go W to a corridor, then N to another one. Go N to find an X'd door. That's the Backstage.
2)Pink Girl (Pinku)- Church/Abyss Intersection: Go to W corridor, then W path to the Church/Abyss. Go S for the effect.
3)Missinghead. (Aka)- Blood Corridor: Go to Blossom Forest (step on pink patch), then go N to a wall. Hug it until you find 2 pillars. Go through to find a Monster Attacking a Town. Go into the House and touch the red girl to warp to the Abyss Maze (A)(it's a one way shortcut). Go E to a hole which leads to the Blood Corridor. Aka glitches things out of existence. Even you can get glitched out of your dream by it.
4)Nightshroud (Kuro)- Office: After getting Aka, go as W as possible in the Abyss Maze and you'll find another hole. Go through to the Company Town, go E to exit the forest, and go N to a large building. Go in to find Kuro

For the Maiko effect

Go to the rainbow goddess/marijuana world and go to the weird little tree thing south of the little statue you have to touch to get there. After going to the tree and teleporting to the weird tree area go to the middle one on your left. You'll be transported to the tatami maze. Find the area with the sakura/cherry blossom door. It's a big-ass area, you can't miss it. Lots of space. So, you go north from there and eventually you'll find her, the Maiko. Tadaaaaaa.

Yume 2kki v.0.09 (b-g)
Changes (that are relevant to effects):
Spring effect added.
Bug maze changed dramatically.
Maiko effect moved to Tatami Maze

Effects:
2)Wolf Effect- Lonely Beach: the Bug Maze is what's changed here. The path through the warp maze is still the same, and can be found in >>2671. The way to get through the Bug Maze is very simple now. Simply go E of the entrance to find a yellow arrow pointing up like usual and follow it over the emptiness until you hit a patch of land again. Go as far up as you can on the tiny patch of land, then go W, until you reach a signpost and use it to warp to the Sign World. From there, look for a blue sign, and it'll warp you to the Warp Maze.

16)Spring- Spring Man's Room: See >>3003 for a picture guide. Or if you want to hear my horrible explanation... First, activate the Library. This is done by either going to the TXT file on the computer or the drawer in the desk next to it, while you are awake. Select the first option to activate the Haunted Library (seriously, look at it's chipset). Once in the Library (accessible from the hub), go to the set of bookshelves immediately on the right of the screen (it loops horizontally and there's only 2 columns of bookshelves). Go to the rightmost book in that column and read it. After you're done, you'll warp to the Apartment Complex, specifically in Abomination's Room. Exit the room, leave the apartment, and go NW of the Apartment building to find an entrance that leads to a separate building. In that other building is a hole in the ground. After going through the hole, you'll be in a hallway. Keep heading E and you'll get warped to a different part of the Hallway, facing W. If you think it warps you back to the same hallway, you are wrong. Go E again and you'll eventually find a staircase. Take it up to get to a part of the hallway with a door. The door leads to Spring Man's Room.

7) Maiko (which the internet informs me means kimono girl, dancing girl, or apprentice geisha)- Tatami Maze: There is a way to get to the Maiko effect by navigating the maze normally, but that's a pain. Instead, go to the Lamppost World, then go slightly SE from the entrance until you find a triple lamp. Touch it and it warps you to the Mirror Lake. Go NW to find a large group of houses, and find the one with the door in its reflection. That warps you to the tatami maze, several rooms W of the effect bearer. First, leave the room south, then go E a bit, then N or so, and you should find the effect in a larger room.

People have asked for bumps, so here is a bump. With an update for Yume2kki.

Invisible effect (you'll need the Chainsaw, the Astronaut and the Penguin effect):

  1. Find your way to the chocolate world (either by the goddess/marijuana world or the park world)
  2. Touch the guy with a conical head.
  3. Walk around until you find the bowling ball (the shortest way is going north from the coneheadguy, then going north and west as much as you can while avoiding dead ends - after some time you'll need to check the south roads too. I'll eventually get a map done).
  4. Get to the top of the map while avoiding the bowling balls - their pattern is always the same, so you just need a bit of memorization.
  5. Now this area is a bit more annoying to navigate... but at least the music is awesome. Wander around until you find a pattern of bowling pins with a triangle made of balls in the middle of it. Touch that triangle.
  6. GODDAMN I FORGOT THIS PART. You'll find yourself at another lane of the bowling world - just make your way to the top of the screen. I don't know if this has to do with my computer or with the game itself, but after you get halfway through, the game is going to get EXTREMELY laggy. Try to not destroy your computer and move to the top.
  7. You'll get to a dark maze (navigate through it if you want, the event at the other end is cool). After you're done here, head out at the entrance.
  8. Teleport maze time. I don't exactly remember how things go here, but after the first warp, using the top warps seemed to work for me. You'll eventually get to a room with a red X at the top wall.
  9. Chainsaw it and use the Astronaut effect to go up from where it was. Keep going up until you find some glass things (if you de-equip the Astronaut effect here, you'll fall all the way down to that gray world).
  10. It's not a maze, so just walk around until you find a purple Urotsuki. You'll notice that trying to approach her causes her to teleport away - so get as near from her as you can, equip the Penguin effect and slide on her direction. Talk to her and EFFECT GET.

Huh, that's strange. I can't get to that warp maze. When I leave the dark area at the end, I just return to the beginning of that lane of the bowling alley. I'm sire I installed the patch... I wonder what's up?

Also, I figured a workable method to get to the second lane of the bowling alley. The guys running around only run in straight lines, so you can use them as makeshift coordinates. Basically, you go E until you see 4 balls running (balls always go down, pins always go West). Then go S until you see 2 pins running. Well, actually you'll probably see the destination before that. Anyways, there you go, a simple way of getting to the second lane.

I'm also making a giant map of all the effects and how to get them for the current version (0.090f) so yeah. It'll be hand drawn because I hate taking screenshots and merging them together.

Yeah, you're right. I killed the girl and was able to get to the warp maze. It seems like there are loops in the maze, but if you go through them enough, it warps you to a different room so the loop breaks. Going through the upper warp all the time seems to be one of those cases and does eventually get you to the x. The other way I got to it was going to the left warp, then right warp 4 times, then the left one.

Wow, I can't believe I forgot about this. It's actually not that hard since the maze is easier to see than before. All you have to do is go E then S down the larger path, then W, and then S first chance you get. The path doesn't branch after that and leads to the giant flower thing from before.

Oh by the way, another thing that I found is, if, after the first bowling lane cleared, you touch the SAME 3 bowling pins, you get to the slightly harder bowling lane, which is to the right of the one you get with the black dot things etc. Also, from the black dot one, you can get to another one to it's left, which is blocked just before the top part of the map. Nothing notable.

Huh, I always assumed it was more like after you clear the first lane, it spawns a shitton of NPCs in the original lane in preparation for you going to the second lane. I've already figured that the entire map is only 3 lanes wide as when you go to the lane left of the second one, you see the first lane again with the million bowling balls running around. Anyways, I doubt it's possible to clear the lane again with the additional NPCs. They crash into each other so often they'll probably form a wall.

Oh hey, individually uploaded. Thanks a lot, these are great. They'd be more helpful if they had the exact route to take, but I guess that'd make everything huge.

I'm still working on my massive all-encompassing map, btw. It'll be done soon. I'll make one for the other fangames after that, I guess.

Edit: There's actually a few things I'd like to mention, Florn, if it's ok.

First, you have to talk to the wisp-like thing first on the way to the spring effect, otherwise you'll be stuck in an endless loop.

You also got some numbering wrong. For example, the rainbow effect's guide has two step 8s.

There's a few more nitpicky things like faster/easier ways to get to certain effects, but I doubt it matters that much. I'll also be keeping them for my own guide. Because I'm a pretentious bastard like that.

How to find the all the effects in the game Lcd Dem

Hopefully you do this in order, it's easiest to do it in order...

  1. Running shoes

What it does: You use it to run faster. Also you can use it to jump over things, like holes and platforms. You will need it, so GET THIS ONE FIRST.
What you need to get it: Nothing.
How to get it: Go to the door gray door, which will lead you to the glowing neon tiles world. Go left, find the heart, follow the trail of pink, boom. Got the effect.

2. Pajamas
What it does: The same thing Medamaude/Multiarms/Eyebomb effect does. Sends you back to the nexus without waking up. Simple.
What you need to get it: Nothing.
How to get it: Just go to flower world and go up. There will be several holes, one of them with pajamas on the ground. Get it. Done.

3. Puppy
What it does: It's like the cat effect in .flow, it allows you to turn into a small dog. It's useful, and you WILL need it to reach some areas, and you WILL need it to get an effect.
What you need to get it: Again, nothing.
How to get it: Go to flower world(green and brown door) and keep looking around. Eventually you will find a room with no flowers, but stones. Go into that door. You will find yourself on a street. Go left, find a dog. Talk to it. It will bark, and give you the effects. Congratulations, you can shrink to a puppy.

4. Space suit
What it does: Abso-fucking-lutely nothing, as far as I know.
What you need to get it: Nada.
How to get it: Just go to the pink block world with the green, one-eyed alien things. Look around for a building. One of them will have an opening on the right side. DON'T go to that one yet. There is no space suit for you. The one with an opening in the front is the one you want. It's pretty straightforward after that.

5. Moon
What it does: I think it's supposed to light up rooms? I don't know. You can change, though, to the waxing phase, and the waning phase. To full moon, then new moon. Basically, on and off. Yeah.
What you need to get it: Zilch.
How to get it: Go to the door beside the flower world door, to underwater world. Find the narrow house with the door and no windows, go inside to find yourself in a green building with metallic floors. Find a staircase and go down it, go right until you find an opening, go inside, have fun with your new moon effect~ :D

6. Fishbowl
What it does: Nothing, really. It makes you a fish bowl with blue shoes. You can make the little fishy do a little flip out of the bowl. Otherwise, it's pretty fucking useless.
What you need to get it: RUNNING SHOES. There will be LOTS of hopping, ladies and gents.
How to get it: Oh god, I fucking hated this. You have to go right after you get the moon effect, then use the dog effect to get inside the small hole at the top of some stairs. Then you find yourself at some spacey place. You don't need the spacesuit, trust me. Just the shoes. Now all I can tell you at this point is to just jump around and explore. The bgm will change at some point, then you'll know that you're on the right track. Good luck.

7. Slug
What it does: Makes you walk through water faster. Basically, Frog effect. Yeah.
What you need: Nothing.
How to get it: Go to prettysparklyfuckingbeautiful world and just wander around, take in the scenery, shit, get high while you're at it. Eventually you'll want to find a big door, and then all you have to do is keep going left until you find what you want. Have fun being a slimy, but pretty, slug. You'll need this later.

8. Some sort of hat...?
What it does: I really have no clue. It's like a shower cap hard hat thing. Does NUHZING.
What you need to get it: Eh. Nothing.
How to get it: This one's pretty straight forward. Just go to the brown door and wander around a little until you find a small cube with a door. Inside the hat thing should be just laying there like "pick me up motherfucker".

9. Flowerbud
What it does: You can bloom or just turn back into a bud. Useless, but hey it's pretty nice.
What you need to get it: Nothing, I think.
How to get it: Uh, same place you go to find the hat thing, brown block world place thing idk. So yeah, find the other door and find yourself at the best fucking place ever. IMO, anyway. You just find the crossroads, a big plus sort of thing, and go down. Then you find the path and go left until you see some wide path that becomes earthy. Go left, I believe, but not down. Eventually you'll find it. Man, I suck at giving directions. But I want you to EXPLORE~~~

10. Small Axe
What it does: Kill things. Yup.
What you need to get it: Nothing.
How to get it: Go to trypophobia maze world, the door with the giant gaping hole in it. Fucking lovely. All you have to do is wonder around a bit to find it, just don't want between anything, or you'll never get to it.

11. Strawberry head thing
What it does: Nothing, but it looks fucking delicious.
What you need to get it: Fuck. I can't remember.
How to get it: I DONT REMEMBER THAT EITHER. D:

12. Kimono
What it does: You wear a kimono. An ugly one at that. But it actually looks nice on Chie.
What you need to get it: The dog effect, you need to be tiny for this one. Also, slug effect.
How to get it: Time to visit the purple lake . Go to where you found the puppy effect, and then you should find another door opening thing. You find it after going left forever, or just a little to the right. Either way you'll get there. Find a little sparkle ball thing and touch it. Like the Pink Lake in Yume Nikki, you'll have to find a red ball thing to get to the place you want. If you have the slug it's not hard. Don't touch the blue one though, it'll be back to square one. Yeah. So, after you find your way to the small opening, it's simple. Turn into a dog and enter. After you enter you don't need to be a dog anymore. So just have fun climbing stairs and finding the cube that gives you the kimono effect. God, that background if fucking pretty. I want to lucid dream that one day, bro.

13. Hoodie
What it does: Nothing. You can put the hood over your head, but that's kind of it. Also that color isn't very flattering on Chie. Juuuuust saying.
What you need to get it: Nothing.
How to get it: Go the the flashy tiles world and go down, then find what seems to be some sort of blue gibberish letters in a box. Go down and find a building you can glitch and walk on. They'll fix it soon. Anyway yeah, go into the door and go right until you find another opening. Now you find a bunch of plants that resemble birds somewhat. Go inside the building and right, then you find some stairs. There's not alot downstairs, just some kid sleeping behind some weeds you can kill. Anyway you shoul go upstairs and keep going upstairs until you get to the roof. Yeah, then you can have a fuss catching the cube by yourself. Have fun with that.

14. Why, those are the instructions! You have that by default. Wasn't that nice of Koronba? :D

Yeah. I'm not specific, but this isn't mean to be that nice and all. Sorry about that, dudes. And I really did forget where the strawberry's supposed to be. Could some one else find that? I'll kick myself later when I realize where it was. For now I think it has something to do with climbing LOTS of latters, and maybe the hell maze. Idk, man.

If I'm not mistaken... in the neon-like world, you need to wander around until you find a circle line thing floating around. Talk to it and you'll be sent to some pretty world, fing a cube with a door there and you'll see a hand, continue right past it and you'll be walking on nothing until you find a strawberry. Or something like that.

Rainbow effect doesnt seem to be obtainable anymore in 0.92.

There is now an impassable cone blocking where the rainbow effect use to be.

EDIT: scratch that, its obtainable in the theater world. Does anyone know where the police effect is?

Okay, here we go...

Dress:
Head over to the apartments from the steel pipe/various pointy things world. Go into the second door, I think it was, and enter the snow world. From that door, go south until you get to the three little cylindrical houses and enter the one in the middle. Get.

Flower:
From the dress effect, head back north, pass the door, and head north some more and slightly west to the lone house. Enter, touch the flower, and exit. South west to the stairs, follow the path until the plants overgrow and block the path. Head through the exit there, touch the vine at the end of the next corridor. Go back to the previous hallway, and head east this time since you can and all. It's pretty linear from there: head to the furthest door to the right, then touch the flower in the middle of the next room. Get.

TV Head:
First, go to the apartment again. All the way through that and up to the checkerboard hospital area. Head to the northern section and keep on until you get to the Famicom World. From the stairs, navigate east through the trees to the "ruins", and just keep on going east until you get to it. Very little fanfare here. Get.

"Tattoo" (the +_+ thing):
Okay, from the nexus, we're going to go to the rave party world. NW to the red and blue street, through the hospital to the bloody thing place and NW again to the red monster and down into the sewers. We're going to head over past the blob of slime and to the rusted pier. West, north, west until you can't anymore, south, take the first turn west again, north, west, north to the streetlight. From the next area, go east, cross the steps, and you'll find yourself on a street. North into the school builing. Pass the kaibutsu behind the counter, head down around the corner to the lower hallway. Head up the stairs and around into the main hall again, then up the last flight of stairs in the middle. Through that door there, speak to the much larger version of the boy you may or may not have seen standing around all over the place already, and get.

School Uniform:
From Tattoo, retrace your steps to the main school staircase, only this time head down into the basement. Up and around and it was either the northern or southern exit there -- whichever one takes you to a larger room full of desks and a couple of female students. Through all the desks and past the two shelves. Speak to the rather familiar girl standing in the middle of the next room. Get.
