I found these theories on the YN Livejournal Community. Some of them are very interesting, although a little far-fetched.
>Madotsuki is actually crazy
Most of her dream is something normal for her, but she was thrown in an asylum at young age. The Toriningen in her dream world represent the nurses, that would most times lock her up in a room and she had to run away from it. They would chase Madotsuki if she was somewhere she wasn't supposed to be, sometimes they would just leave her alone. If Madotsuki did something suspicious (in the dream world, this would represent poking or stabbing the pacific Toriningen), they would chase her again. She wanted freedom, so she was able to run away from the asylum and get into an abandoned apartment. All she can do is play videogames and sleep, and she refuses to even unlock the door, because she knows she will be taken to the asylum if she leaves. She is only able to open the door in her dreams, which represent her wish for freedom; even if she ran away from the asylum, she wouldn't be able to go out, or the 'bad nurses' would catch her. The Toriningen having weird eyes and appearence is Madotsuki's way of saying, "I'm not crazy. The people who think I'm crazy are the crazy ones."
>Seccom Masada-Sensei
Having the 'sensei' honorific in his name (at least I've seen him being called like this a lot), I believe Masada is someone Madotsuki admires. The Spaceship to Mars might represent Madotsuki's childish dream of going into space, and Masada might be a spaceman that Madotsuki likes. The fact that his face is twisted, however, might mean that he paid little or no attention to her, or actually mistreated her. He being all black and white might mean that he's someone Madotsuki fears, or someone who's dead.
>Monoko (The Five-armed girl)
She's wearing something that looks like a school uniform, so she might be one of Madotsuki's school or asylum mates. I believe that any 'human' or slightly human being in Madotsuki's dreams are troubled people like her, that she met in real life. She seems to be a normal monochromatic girl, until you use the stoplight. She grows two more arms, connected to her normal arms, and grows one hand in her head.She has some kind of void in her belly and her eyes also change, one of them seems to be leaking out of her face. I believe she has a case of lost, parasite twin (see Parasitic twin and Fetus in fetu at Wikipedia to see what it is). She might have the arms from a twin that was never born, and didn't manage to have a surgery to remove them. Seeing as she's black and white, she might be already dead.
>Monoe
She's also black-and-white and wear a school uniform. If you follow the 'Madotsuki is crazy' theory, this would mean Monoe also wanted to run from the asylum, and got something like a 'ticket' to get out of reach of the asylum nurses and staff. But she's got only one, and her dillema is, who leaves the place? That's why she randomly teleports Madotsuki or herself; she wants they both to leave together, but only one can go. This theory is also good if they were both raped, and Monoe and Madotsuki want to leave the town, but again, only one can leave. Again: the fact that she's monochromatic might mean that she's already dead, wheter she left the asylum or not.
>Poniko and Uboa
Poniko is the only human (that I have seen, correct me if I'm wrong) that is not monochromatic, though her 'other form' is. The fact that Uboa is black and white might mean that he's someone Madotsuki fears. Poniko is a sweet girl most of the times, and might be a friend of Madotsuki. Poniko being in such a far and enclosed place might be Madotsuki trying to shut her from the rest of her dream world, meaning that she 1) really likes Poniko or 2) is really afraid of Poniko. Either way, most times when Madotsuki talked to Poniko, she was a nice girl. But sometimes, she would be really mean, not actually evil, but something like bullying and saying mean things, as Uboa doesn't really do anything unless you stab him. When Uboa appears in Madotsuki's dream, she gets stuck inside, which means at some point the bad side of Poniko locked her in a room, and could only get out if she begged(interacting with Uboa in-game). However, Madotsuki pissed Bad Poniko off, and she did something awful to punish Madotsuki. This is represented by the "Uboa's Demention" area. What did Poniko do to Madotsuki? Since the area is full of water, and Uboa is underwater, I think it's safe to assume that Poniko tried to drown herself with Madotsuki.
>The Building
The one building you can fly off with the Witch effect. It seems to be the only normal place in the dreamworld, and it might represent the asylum. Madotsuki uses the Witch to run away from the asylum. Cancelling it midair causes Madotsuki to fall off bed and wake up, that might mean that she believes that it is impossible to run away.
>The hands
There are many hands in the game, like in the staircase to the Poop Hair and in the Eyeball World, what do they mean? They might be the hands of the nurses and doctors, trying to reach Madotsuki and tie her down at the asylum again.
>The Ending
The ending is creepy. I tried to think why the effects turn into eggs when you throw them away in the Nexus, and I actually just came up with something. The effects might be somewhat the 'children', and the 'source' of Madotsuki's madness, that are there for her entertainment. She decides to remake her thoughts, and puts them into eggs, that represent the recreating of her own mind. But only by laying all of the 'sources' she will be able to 'undo' her crazyness. By throwing all of them away and waking up, she decides to go back to the asylum and show everyone that she's fine now. Jumping off the balcony with the stairs is just to show that she's finally free. The bloodstain represents that her dream world is dead.
There are, of course, variations.
She might have actually killed herself, for noticing that without her dreams, she was nothing. When she died, she went back to her dream and kept living in her 'perfect world'. Even if it's creepy, it's her home.
Another one! She might have just assumed that by disposing of her toys, she would be clean. She didn't want to believe that the madness was on her head, and the source of it were the effects she collected. She went back to the asylum, but died there, still crazy, trying to find a way to take everything from her head.
